It is encouraged for parents to register into the Rank One Parent Portal, but not mandatory. As a registered member, you will have access to: online forms, your student’s status, get push notifications and ability to download completed paperwork. You can also download the Rank One app for your phone.

Both parent/guardian and the student athlete should complete these forms while present together. You will need your student’s ID number.

1. Cole Athletic Form (All student ID #s are 6 digits. If yours is not, please put preceding “0s” before your ID # to make it 6 digits)

2. Medical History

REQUIRED FORMS TO BE PRINTED

1. Asthma Action Plan (Must only be filled out if asthmatic or uses inhaler)

2. Medical History Form/ Preparticipation Physical Evaluation Form (This form is REQUIRED for all students in incoming grades 7, 9, or 11 wanting to participate in athletics, cheer, or band regardless of having a previous physical on file in the previous year.)

INSTRUCTIONS ON HOW TO PRINT THE FORMS

• Click on the Download and Print tab.

• Print the Asthma Forms (Must only be filled out if asthmatic or uses inhaler)

• Print Physical & Medical History Form (To be completed by a medical provider for incoming grades 7, 9, and 11, or if you do not previously have a physical/medical history form on file.)

All forms must be completed, submitted and on file prior to participation in any camp, try-outs, practices, scrimmages and/or contests before, during or after school.
